The “Five Wishes Living Will” is a legal document and will help you express how you want to be treated if you are seriously ill and unable to speak for yourself. It is unique among all other living will and health agent forms because it looks at all of a person’s needs: medical, personal, emotional and spiritual. You will receive a free Five Wishes document, instructions to complete it, time to ask questions, and an opportunity to acquire additional original documents for friends and loved ones.
“Five Wishes” lets your family and doctors know:
Which person you want to make health care decisions when you can’t make them for yourself.
The kind of medical treatment you want or don’t want.
How comfortable you want to be during your illnesses and your last days.
How you want people to treat you.
What you want your loved ones to know and what to do to plan your memoriam.
We will also spend a few minutes introducing the concept of hospice care and how it serves both patients and their families. We will also display and offer brochures / flyers/ information about all of our agency services in Snohomish County.
